Every one of these is something I have actually been called in to fix, and most of them have a case study on this site showing exactly how it ended. If more than one applies to you, that is not embarrassing. It is just how most systems end up once real life gets at them.
Does your team work inside wp-admin all day because nobody ever built them a real screen?
